Caregiver Services:
We'll provide the services outlined in the Care Plan we've discussed. We'll agree on the days, times,
and duration of services together.
Selection of Services:
Careoxy Healthcare Services Private Limited representative explained the services thoroughly
to me or my next of kin.
We discussed the differences between service categories and what each includes.
Shift timings were agreed upon during the onboarding process.
Every new admission will follow our standard care assessment and supervision process.
We'll assign staff based on care needs and cover for their time off or emergencies with other
staff.
If needed, services may be paused until alternative arrangements are made.
Services will be provided based on the advance booking and agreement.
Payment for Services:
I'll pay the estimated amount in advance and provide recurring deposits for future services.
Costs may change based on changes in my condition.
I'll pay extra for any supplies used by the staff during care.
Transportation charges may vary if I travel to different locations with staff.
If a staff member is injured during service, I'm responsible for their treatment.
Any equipment provided by Careoxy Healthcare Services Private Limited will incur costs, and
I'm responsible for its proper use.
All charges must be paid before discharge.
If payment isn't made, services may be stopped until payment is received, and I'll receive
updates on my payment status.
Monitoring and Follow-up:
My service needs will be reviewed by a supervisor as needed, and services may change
accordingly.
Medical Records:
The Nursing Supervisor will collect relevant data at the end of the service and provide a closure
report to me or my representative.
Agreement Terms:
Duration: This agreement lasts for one year from the Effective Date or until services are
completed, whichever happens first unless extended or terminated.
Payment: The Beneficiary pays for services in advance through card/online payments to
Careoxy Healthcare Services Private Limited. No cash is given directly to staff without
authorization.
Fees: The quoted professional service fee includes taxes but not goods and services tax.
Service Scope: Careoxy Healthcare Services Private Limited staff only provide agreed-upon
services. Any changes to the agreement must be in writing and signed by both parties.
Amenities: The Beneficiary provides basic amenities for staff stay at their expense.
Staff Responsibilities: Staff won't perform domestic chores. The Beneficiary must provide food,
rest, and treat staff with respect.
Leave and Replacement: Staff follow Indian labor laws for leave. Careoxy Healthcare Services
Private Limited may replace staff if necessary.
Termination: Either party can terminate the agreement with 24-hour notice or mutual consent.
Termination can also occur for non-performance or breach of terms.
Refunds: Upon early termination, owed payments become due, and any balance is refunded on
a pro-rata basis. Will be refunded within 7 working days.
Replacement: It can take up to 12hrs to 24hrs of time if there is a requirement for the
replacement.
Non-Compete: The Beneficiary can't hire away Careoxy Healthcare Services Private Limited staff
for 12 months after the agreement ends, or damages will be sought.
Agreement Validity: The agreement ends upon the Beneficiary's death, with no refunds possible.
Jurisdiction: Indian laws govern the agreement, and disputes are subject to New Delhi courts.
Arbitration: Disputes are resolved through arbitration in Delhi, conducted in English. Costs are
shared unless otherwise decided by the arbitrator.

Terms and Conditions:
Safety: Beneficiaries accept risks associated with services and release Careoxy Healthcare
Services Private Limited from liability.
Control: Careoxy Healthcare Services Private Limited isn't liable for actions beyond its control or
those under the Beneficiary's supervision.
Indemnity: Beneficiaries indemnify Careoxy Healthcare Services Private Limited for any claims
arising from their negligence.
Authorization: Beneficiaries consent to treatment, release of medical records, and
communication with doctors.
Consent: Beneficiaries agree to treatment, acknowledge responsibility for outcomes, and allow
the use of photos/videos for educational/promotional purposes.
Emergency Treatment: Beneficiaries authorize Careoxy Healthcare Services Private Limited to
provide or obtain necessary medical treatment during emergencies.
The Beneficiary confirms understanding of these terms after adequate explanations.
I acknowledge that Careoxy Healthcare Services Private Limited authorities are not responsible for
any untoward incidents due to my decision.
